Medical and
Wellness Services:
- Iowa Senior Health Program--55 and older
This is a program designed to promote and maintain the health of older adults. An ISHP
screening clinic provides these services: a detailed health history, a nursing physical
assessment, selected screening test, i.e. visual, hearing , blood & urine, and blood
pressure. The screening will take approximately 2 hours and is available by appointment.
The ISHP program is provided through a grant from the Iowa Department of Public Health so
there is no fee, but a donation will be accepted. Please call 737-2971 for an appointment.

Homemaker
Services |
Homemaker Home
Health Aide services are provided to clients in their homes to enhance their capacity to
attain or maintain their independence. They are provided when their independence. They are
provided when the homemaker experiences limitations, stress, or crisis. Homemaker services
will be provided to clients in their home in a 30 mile radius of the Sioux Center
Hospital. All clients are evaluated for appropriate care on an individual basis. |

CHEARS Home Care |
Home Health Care
services are provided to patients and their families in their home settings. Care is
provided to patients of all ages who are disabled, chronically ill, or to those who are
recovering from an illness or surgery. Services are provided to patients residing in a 30
mile radius of the CHEARS office. CHEARS is a licensed and certified provided of home
health care. Many services are paid for by Medicare, Medicaid, or other insurance
carriers. An initial evaluation visit is made free of charge. CHEARS will provide services
regardless of a family's ability to pay. |

CHEARS Hospice |
CHEARS Hospice
offers a coordinated program of services to terminally ill patients and their families.
Care is provided in a patient's home as well as the hospital and nursing home setting.
Physical, emotional, social and spiritual care is provided by a hospice team which
includes a physician, nurse, home health aide, social worker, pastor, counselor, and
volunteers. Hospice care is supportive care for the patient and family, focusing on
managing the pain and symptoms of the disease as well as providing spiritual and emotional

New Homestead Community Room &
Senior
Center:
- Two party rooms are located at the New
Homestead at 510 North
Meadow Drive. These rooms can be rented for group gatherings of 100 people
or less. A stove and refrigerator are available. A fee is charged per room
and can be reserved by calling 712-722-2237.
All Seasons
Center Senior Center:
- The Senior Activity room is now available for your use. This room can
be used by community seniors for social gatherings, games, and other
activities or just to get together for coffee. We welcome your suggestions on what activities you would
like.
Dinner Date:
- These
fellowship dinners are funded in part by the Northwest Aging Association and
administered by community volunteers. Dinners
are served weekly on Tuesday evenings at 5:30 p.m. in the
Senior Center at the New Homestead. The meals meet 1/3 of daily RDA and all health and safety regulations. Contributions are voluntary
within the recommended range of $3.50 - $5.50. Please pay what you feel is
fair and affordable; but please remember your contributions and attendance are needed to
insure the continuation of this program.
Please call (712) 722-3123 for reservations and leave a message with your
name and number attending. Reservations need to be called in by 11:00 a.m. Monday if you
plan on attending Tuesday evening. For more information about Dinner
Date, call (712) 722-2237. All meal coupons need to be purchased in
advance at American State Bank (prior to attending the meal) during regular
business hours. Only a
BLUE coupon will permit you to receive a meal. NO late reservations,
walk-ins or cash payments at the door will be accepted. Following dinner,
seniors are invited to stay and play cards or games. Remember your
contributions and willingness to volunteer are needed to insure the
continuation of the program.
Home Delivered Meals:
The
Meals on Wheels program provides an often much needed noon meal to elderly
and shut-ins (60 years of age or older) of the community. To have a
meal delivered call (712) 722-2237 before 11:00 a.m. the day BEFORE you
would like the meal delivered. GREEN coupons for Meals on Wheels are
available at the New Homestead office during regular morning office hours.
The meals meet all health
and safety regulations. These noon meals are delivered to homes five days
a week (Mon-Fri) at noon for a cost of $4.00 per meal.
This program is funded with contributions from local supporters and
serviced by volunteers of the community. It is great to live in a
community where people help one another. If you would like to volunteer to
assist this program or make a contribution to supplement the meal
expenses, please contact the Sioux Center Meals on Wheels at (712) 722-2237.

Foster Grandparent
Program:
The Foster Grandparent program offers
seniors age 60 and older opportunities to serve as mentors, tutors, and
caregivers for children and youth with special needs. They provide 20
hours of weekly service to community organizations such as schools,
hospitals, and youth centers. The Foster Grandparents must meet income
eligibility requirements to receive a modest tax free stipend for their

Senior Companion Program:
If you are or know someone who is 60 years old or older,
earns $1495 per month or less, likes to assist other seniors, and is looking
for something worthwhile to do---The Rock Valley Rotary Senior Companion
Program is the Program for you!Senior Companions enable care recipients
to remain independent by providing assistance with non-medical care such as
meal preparation, laundry, shopping, light housekeeping, active listening,
and companionship. We offer lower to middle income individuals over 60 an
opportunity to volunteer one-on-one with homebound seniors in *YOUR LOCAL
COMMUNITY*.In return for volunteering 15 - 40 hours a week, Senior
Companions receive:
· $2.65 per hour stipend or approximately $220.00 per month (Tax free
stipend does NOT affect other benefits received)
· Reimbursement of Transportation mileage to and from volunteer site
· Pre-service and On going Training
· An Annual physical paid by the Program

The New Homestead
The Sioux Center Low Rent Housing
Agency ( New Homestead) is an affordable, low rent, public housing program
administered by HUD guidelines. The New Homestead is committed to
excellence in assisting low income seniors and families with drug-free,
safe, decent and affordable housing opportunities.
The elderly project consists of 50 one-bedroom and 6
two-bedroom units of housing in a quality open setting located on Meadow
Drive. These apartments are constructed of brick with each unit having a
front/back outside entrance, living room, kitchen (appliances furnished),
bedroom and bath. Rent is 30 % of the adjusted family income or a maximum
of $360 per month, whichever is less. Other added services include:
full-time maintenance, snow removal, lawn mowing, utilities included in
rent, free laundry facilities, home delivered meals, social room, etc.
The family project consists of 18
scattered family dwellings with two, three, or four bedrooms within the
Sioux Center community. Rent is 30% of the family income with a maximum
according to bedroom size.

Franken Manor is an affordable senior living community that
promotes independence, security and well-being. With service
attitudes grounded in Christian values, Franken Manor is a
community owned senior apartment complex offering independent and

Regency Retirement

This locally sponsored, nonprofit corporation is an independent,
affordable life-style for people over 62. The board of directors is
elected with representation from residents and sponsor. The Regency
features spacious living, with one and two bedroom styles, furnished
kitchens, individually controlled heat and air conditioning, ample
closet and storage space, entry security system, no maintenance, garage
parking, and walk out patios. Other features include: van
transportation, optional dining, library/lounge, great room/dining room
and craft room.

Hospital and Health Center. Sixty-nine residents reside at the
Medicare/Medicaid certified skilled nursing facility attached to Crown
Pointe. This facility
offers the following services: social services, activities, physical
therapy/restorative therapy, van utilization, social outings, and

Kosgrove Estate
Kosgrove offers personalized assisted living care in a family type
environment. Base rent includes: weekly housekeeping and flat linen
laundry, 3 "home cooked" meals per day served family style,

Taxi
Service:
- A taxi service is available to all
Sioux Center residents at a rate of $3.00 per ride one way. Tickets are available
from the RIDES driver at $3.00 each. Service is available Monday through
Friday, 8:00 a.m. - 5:00 p.m. and can be scheduled by calling
1-800-358-5037. Taxi service is also available to and from the Sioux Center
Airport at $3.00 one way per person per trip.
Discounted tickets are available for qualifying Senior Citizens by
calling Northwest Aging Association at 1-800-242-5033.

Library Service The library offers their complete library service to anyone who is
housebound. You may call the library at 722- 2138 for home deliveries. A
librarian also visits the New Homestead, Franken Manor, the Regency,
Crown Pointe and Royale Meadows on a regular schedule.
